Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to CodePath.org (the “Organization”), and its employees, agents, licenses, successors, and third-party organizations all ownership rights and irrevocable right and permission to use my image and likeness and/or any written or recorded statements from me to copyright, publish, sell, distribute, and/or promote the recorded video, photo, interview, and/or audio to use in its publications, advertising or other media activities (including the Internet). 

This consent includes, but is not limited to:

• Permission to use my photograph;
• Permission to use my first name; and
• Permission to use written statements provided to CodePath.org for inclusion in its publications, in newspapers, magazines and other print media, on television, radio and electronic media (including the internet), in theatrical media and/or in mailings for educational and awareness.

Further, I agree to indemnify and hold harmless CodePath.org from and against any and all claims and demands whatsoever, including but not limited to claims or demands arising from infringement of copyright, proprietary right of claims of libel, obscenity or invasion of privacy that may arise out of any part of my participation in said program or from any breach or warranties or representations herein contained.

I understand that my participation is voluntary and that I may, at any time, discontinue my involvement before signing this document. If I choose to discontinue participation, I will notify the principal parties of CodePath.org by providing written notice.

This consent is given in perpetuity, and does not require prior approval by me.
